Resolute Shines Spotlight on Medical Home Network’s Community Paramedic Partnership
Resolute recently secured a feature story on WTTW’s Chicago Tonight about Medical Home Network’s (MHN) partnership with the Chicago Fire Department (CFD) to connect CFD paramedics to patients suffering from long-term health conditions.
The program, which is the first of its kind in the country, aims to reduce emergency room visits and enable patients to recover from the home. Paramedics visit patients for up to four non-emergency visits, where they evaluate patients’ medications, answer questions about their conditions, examine health and safety hazards, and determine if there are social detriments of health slowing recovery. The paramedics also connect directly with patients’ doctors to discuss treatment plans and communicate patients’ needs. Patients involved in the program have already reported improved overall health and decreased emergency room visits, reducing the strain on the healthcare system. The five-minute segment highlights Denise Jackson and Rosa Gonzales, two patients who have graduated from the program and have seen tremendous improvements to their health since working with the community paramedics.
